MAKING A REFERRAL

Parents, adult children or other family members can refer a loved one to the Northeast Center for Rehabilitation and Brain Injury.

Discharge planners, case managers, social workers, physicians or anyone having an interest in supporting the care and rehabilitation of an individual can also make referrals.
