Stroke prediction and stroke prevention with atorvastatin in the Collaborative Atorvastatin Diabetes Study CARDS

Patients with Type 2 diabetes have an elevated risk of stroke. The role of lipid levels and diabetes-specific factors in risk prediction of stroke is unclear, and estimates of efficacy of lipid-lowering therapy vary between trials.

Magnetic Resonance Imaging of Individual Cerebral Perfusion Territories Improves the Diagnosis of Embolic Stroke

This case study demonstrates the feasibility and diagnostic value of the oblique-plane arterial spin labeling method to visualize individual perfusion territories in a patient with embolic stroke. The perfusion territories obtained with this technique differed significantly from the standard anatomical situation. Imaging findings suggested that all acute embolic lesions in both hemispheres had arisen from one symptomatic internal carotid artery stenosis.

The International Subarachnoid Aneurysm Trial (ISAT) reported that endovascular coiling yields better clinical outcomes than surgical clipping at 1 year. The high cost of the consumables associated with the endovascular coiling procedure (particularly the coils) led health care purchasers to conclude that coiling was a more costly procedure overall. To examine this assumption and provide evidence for future policy, accurate and comprehensive data are required on the overall resource usage and cost of each strategy.

Disruption of Limbic Pathways in a Case of Profound Amnesia

We report a case of episodic amnesia in which the anatomical basis of injury was investigated by diffusion tensor imaging (DTI). Two months after an adult male suffered severe closed head injury, conventional magnetic resonance imaging (CMRI) revealed only a right superior frontal lesion. However, 14 years later, DTI revealed structural anomalies not visible on CMRI involving limbic white matter tracts, notably the fornix, which could explain the amnesia.

The pathogenesis of cerebral ischemia associated with subarachnoid hemorrhage (SAH) still remains elusive. The aim of this study was to examine the involvement of mitogen-activated protein kinase (MAPK) and protein kinase C (PKC) subtypes in the pathophysiology of cerebral ischemia after SAH in cerebral arteries and microvessels and to examine temporal activation of the kinases. We hypothesize that treatment with a MAPK or PKC inhibitor will prevent the SAH-induced kinase activation in brain vessels.

Anti-flatulence treatment and status epilepticus - a case of camphor intoxication

We describe a case of a young child who lived in Hong Kong who presented with a severe epilepticus status after a return flight to Paris. Routine laboratory tests failed to establish a cause. Upon further questioning, the parents reported that the nanny had given an abdominal massage to the child with an unlabelled solution reported to have anti-flatulence effects. Toxicological analysis of this solution revealed the presence of camphor.

The Minimum Data Set (MDS) is a standardized, federally mandated assessment tool used to identify the needs of nursing home residents and rehabilitation and nursing services received on a routine basis. These data can be used to identify people with traumatic brain injury (TBI), where a diagnosis is recorded. However, both limitations of the MDS and the presence of other neurological issues that can be confused with TBI can reduce the likelihood that brain injury is identified where it exists. The article discusses innovative methodological approaches and challenges to using the MDS data to identify people with TBI in nursing homes.

Marginal sinus fistulas (MSFs) are uncommon vascular anomalies. Occasionally, the dominant venous drainage is forced retrograde up the inferior petrosal sinus and into the cavernous sinus, causing chemosis, proptosis, and ocular hypertension, mimicking a carotid cavernous fistula. This atypical clinical presentation may lead to misdiagnosis and inappropriate hazardous treatment of an MSF. Identifying the site of the fistula and understanding the anatomy of the venous drainage are critical in providing appropriate, safe, and efficacious endovascular treatment.

West Nile virus (WNV) was initially isolated and identified in 1937 in Uganda (Africa). Although the most common source in these cases is the lung, determining a primary source is difficult on routinely stained slides. Immunohistochemical stain panels including differential cytokeratins, hormone receptors, and breast-specific proteins are commonly used in these cases. Recently, attention has turned to tissue-specific transcription factors, such as thyroid transcription factor 1 (TTF-1) and Cdx2, in the appraisal of metastatic adenocarcinomas.

Age related white matter changes (ARWMC) are frequent in non-demented old subjects and are associated with impaired cognitive function. Our aim was to study the influence of vascular risk factors and ARWMC on the neuropsychological performance of an independent elderly population, to see if vascular risk factors impair cognition in addition to the effects of ARWMC.

Anthropometric approximation of body weight in unresponsive stroke patients

Thrombolysis of acute ischaemic stroke is based strictly on body weight to ensure efficacy and to prevent bleeding complications. Many candidate stroke patients are unable to communicate their body weight, and there is often neither the means nor the time to weigh the patient. Instead, weight is estimated visually by the attending physician, but this is known to be inaccurate.

A history of loss of consciousness (LOC) or post-traumatic amnesia (PTA) is commonly considered a prerequisite for minor head injury (MHI), although neurocranial complications also occur when LOC/PTA are absent, particularly in the presence of other risk factors. The purpose of this study was to evaluate whether known risk factors for complications after MHI in the absence of LOC/PTA have the same predictive value as when LOC/PTA are present.

Glossoplegia in a small cortical infarction

We recently observed a patient with obvious contralateral tongue deviation with minimal lower facial paresis caused by a small cortical infarction confirmed by MRI. The small cortical lesion, causing obvious tongue deviation in our patient, was located lateral to the precentral knob which is known as a reliable anatomical landmark for the motor hand area.
