What is it like to be vegetative or minimally conscious
Current Opinion in Neurology. 20(6):609-613, December 2007
Patients in a vegetative or minimally conscious state continue to pose problems in terms of diagnosis, prognosis and treatment. Despite recent waves of international media attention following Terri Schiavo's death and the 'miracle recovery' of Terry Wallis, research efforts aimed at increasing our knowledge about brain function in these conditions remain scarce and must address a series of difficulties, including financial and ethical barriers. Phosphorylated Pak1 Level in the Cytoplasm Correlates with Shorter Survival Time in Patients with Glioblastoma
Clinical Cancer Research 13, 6603-6609, November 15, 2007
Glioblastoma is the most common primary malignant tumor in the brain. It aggressively invades the surrounding parenchyma, often allowing the tumor to progress after surgery. Accumulating evidence has shown that phosphorylated p21-activated kinase 1 (Pak1), a mediator of small guanosine
triphosphatases, plays a role in the proliferation, survival, and invasiveness of cancer cells. Thus, we examined patterns of Pak1 expression in glioblastoma and sought to determine whether the level of phosphorylated Pak1 in glioblastoma cells is associated with patient survival time.

Challenges and Approaches to the Identification of Traumatic Brain Injury Among Nursing Home Residents
Journal of Head Trauma Rehabilitation. Screening and Identification of
TBI. 22(6):350-359, November/December 2007
The Minimum Data Set (MDS) is a standardized, federally mandated assessment tool used to identify the needs of nursing home residents and rehabilitation and nursing services received on a routine basis. These data can be used to identify people with traumatic brain injury
(TBI), where a diagnosis is recorded. However, both limitations of the MDS and the presence of other neurological issues that can be confused with TBI can reduce the likelihood that brain injury is identified where it exists. The article discusses innovative methodological approaches and challenges to using the MDS data to identify people with TBI in nursing homes.

Setting an alert for tick-borne encephalitis
British Journal of Nursing, Vol. 16, Iss. 19, 25 Oct 2007, pp 1176
Headache, fatigue, fever, myalagia or catarrh. If a patient walks in to your surgery complaining of these symptoms, it is usual to ask if there has been any overseas travel when taking the history. But if your patient has only been in Europe does this mean that their condition is not travel-related? Well, one illness that can present with these signs is tick-borne encephalitis
(TBE).

Pediatric stroke - current developments
Current Opinion in Pediatrics. 19(6):657-662, December 2007
Pediatric arterial ischemic stroke is relatively rare but carries a considerable impact and high risk of recurrence - especially in patients with sickle cell disease and various forms of
vasculopathy. We will discuss risk factors, vascular physiology, and primary/secondary/rehabilitative therapies. Goals for future investigation and treatment are suggested.

Spinal trauma - A challenge ahead
Indian Journal of Orthopaedics Year : 2007 Volume : 41 Issue : 4 Page : 253-254
The spinal cord and cauda equina are injured by disruption of neural tissues by violation of the spinal canal by bony missiles (pieces) entering into the spinal canal as a result of impact. The acute compression of the spinal cord produces histologically appreciable and irreversible secondary changes in the spinal cord.

Marginal Sinus Arteriovenous Fistulas Mimicking Carotid Cavernous Fistulas - Diagnostic and Therapeutic Considerations
American Journal of Neuroradiology 28:1915-1918, November-December 2007
Marginal sinus fistulas (MSFs) are uncommon vascular anomalies. Occasionally, the dominant venous drainage is forced retrograde up the inferior petrosal sinus and into the cavernous sinus, causing
chemosis, proptosis, and ocular hypertension, mimicking a carotid cavernous fistula. This atypical clinical presentation may lead to misdiagnosis and inappropriate hazardous treatment of an
MSF. Identifying the site of the fistula and understanding the anatomy of the venous drainage are critical in providing appropriate, safe, and efficacious endovascular treatment.

Acute subcortical stroke and early serotonergic modification - a IDAP study
European Journal of Neurology, Volume 14, Number 12, December 2007 , pp. 1378-1382(5)
The intensity dependence of the auditory-evoked potentials (IDAP) is inversely related to serotonergic tone. Depression is frequently observed after stroke, associated with cognitive impairment and increased mortality. Aim of this study was to investigate the serotonergic tone in acute stroke patients by
IDAP.

Update on new treatments and developments in childhood brain tumors
Current Opinion in Pediatrics. 19(6):670-674, December 2007
Childhood primary central nervous system tumors remain a therapeutic conundrum. As the second most common pediatric cancer, brain tumors lead to significantly worse survival and long-term effects compared with those seen with hematologic malignancies and other solid tumors. This review discusses current management strategies in three pediatric brain tumors, the long-term effects of therapy, as well as novel laboratory findings that may alter future treatment strategies.

Levels of Stress and Intervention Preferences of Caregivers of Brain Tumor Patients
Cancer Nursing. 30(6):E33-E39, November/December 2007
The purpose of this pilot study was to assess the level of stress experienced by caregivers of brain tumor patients and to examine both their interest in and preferences for stress reduction programs. Using a convenience sample of 60 adult caregivers, we distributed a study questionnaire that examined the caregivers' level of stress, beliefs, past experiences, and preferences in regard to stress reduction programs.

High risk of pituitary dysfunction due to aneurysmal subarachnoid haemorrhage - a prospective investigation of anterior pituitary function in the acute phase and 12 months after the event
Clinical Endocrinology, Volume 67, Number 6, December 2007 , pp. 931-937(7)
Recent investigations have provided evidence for a high prevalence of pituitary dysfunction in patients with subarachnoid haemorrhage
(SAH). However, apart from one study, all of the previous data were obtained from retrospective studies. To our knowledge there is no previously reported study in which pituitary function was investigated in the same patients immediately after SAH and 12 months later.

What constitutes high quality of care for adults with epilepsy
NEUROLOGY 2007;69:2020-2027
Providers are increasingly being held accountable for the quality of care provided. While quality indicators have been used to benchmark the quality of care for a number of other disease states, no such measures are available for evaluating the quality of care provided to adults with epilepsy. In order to assess and improve quality of care, it is critical to develop valid quality indicators.
