This review is a summary of the New Zealand guideline for the management of Transient Ischaemic Attack (TIA). TIA is a medical emergency and warrants urgent attention. The risk of early stroke following TIA may be as high as 12% at 7 days, and 20% at 90 days, with half of these strokes occurring within the first 48 hours.

We report the case of a 6-year-old female who suffered a left hemisphere stroke attributed to a genetically determined prothrombotic state. She presented a fluent speech pattern with selective difficulty in retrieving names but not verbs. More than 200 years ago, several thousand people intermittently immigrated to North America from Switzerland and Germany to found small religious communities. To this day, their descendants, who are now known as Amish-Mennonites, follow their forebears' practice of within-community marriage to preserve their way of life, and their populations have increased to present-day levels of about several hundred thousands.
