Ventilation heterogeneity in well controlled asthmatic children, with normal spirometry, indicates residual airways disease

Thorax. Published Online First: 4 August 2008. doi:10.1136/thx.2007.095018

In adults with asthma, ventilation heterogeneity, independent of inflammation, has been hypothesised to be associated with airway remodelling. Bronchial biopsy in preschool children with wheeze demonstrates early structural changes. Ventilation heterogeneity is sensitive to airway disease in other paediatric respiratory conditions (cystic fibrosis), so may be sensitive to early airway disease in asthma.

Abnormalities of Respiratory Control and the Respiratory Motor Unit

Neurologist. 14(5):273-288, September 2008

Control of ventilation depends on a brainstem neuronal network that controls activity of the motor neurons innervating the respiratory muscles. This network includes the pontine respiratory group and the dorsal and ventral respiratory groups in the medulla. Neurologic disorders affecting these areas or the respiratory motor unit may lead to abnormal breathing.
