Increasing Genetic Relatedness of Ciprofloxacin-Resistant Streptococcus pneumoniae Isolated in Canada from 1997 to 2005{triangledown}

Antimicrobial Agents and Chemotherapy, March 2008, p. 1190-1194, Vol. 52, No. 3

The genetic relatedness of ciprofloxacin-resistant Streptococcus pneumoniae isolates collected from 1997 to 2002 (n = 82) and 2003 to 2005 (n = 123) was compared by pulsed-field gel electrophoresis (PFGE). Increased genetic homogeneity among the isolates from 2003 to 2005 (cluster analysis; P < 0.001) appeared to be due to expansion of existing clonal groups and to introduction of new PFGE types.

Antimicrobial Agents and Chemotherapy, March 2008, p. 822-830, Vol. 52, No. 3

The low prevalence of ciprofloxacin-resistant (Cpr) Streptococcus pneumoniae isolates carrying recombinant topoisomerase IV genes could be attributed to a fitness cost imposed by the horizontal transfer, which often implies the acquisition of larger-than-normal parE-parC intergenic regions.

Dectin-1 Fc Targeting of Aspergillus fumigatus Beta-Glucans Augments Innate Defense against Invasive Pulmonary Aspergillosis

Antimicrobial Agents and Chemotherapy, March 2008, p. 1171-1172, Vol. 52, No. 3

Invasive pulmonary aspergillosis (IPA) has significantly increased over the last decade. Here, a fusion protein consisting of the Dectin-1 extracellular domain linked to the Fc portion of murine immunoglobulin G1 augmented alveolar macrophage killing of Aspergillus fumigatus and shifted mortality associated with IPA via attenuation of A. fumigatus growth in the lung.

Current Opinion in Infectious Diseases. 21(2):174-178, April 2008

Diagnosis of ventilator-associated pneumonia remains controversial. Different approaches are advocated, but none has yet demonstrated superiority. Diagnosis based on clinical data and aetiological diagnosis of ventilator-associated pneumonia episodes are two concepts that should be combined in an integrative evaluation for ventilator-associated pneumonia. Recent findings in diagnosis are reviewed here.

Multiple-breath inert gas washout and spirometry versus structural lung disease in cystic fibrosis

Thorax 2008;63:129-134

A sensitive and valid non-invasive marker of early cystic fibrosis (CF) lung disease is sought. The lung clearance index (LCI) from multiple-breath washout (MBW) is known to detect abnormal lung function more readily than spirometry in children and teenagers with CF, but its relationship to structural lung abnormalities is unknown. A study was undertaken to determine the agreements between LCI and spirometry, respectively, with structural lung disease as measured by high-resolution computed tomography (HRCT) in children and teenagers with CF.

Antimicrobial Agents and Chemotherapy, March 2008, p. 1028-1033, Vol. 52, No. 3

Carbapenem-resistant Klebsiella pneumoniae (CRKP) is an emerging nosocomial pathogen. Little is known about its risk factors or mortality. We performed a case-case-control study to assess the risks for CRKP isolation and a retrospective cohort study to assess mortality in three groups of hospitalized adults: (i) patients from whom CRKP was isolated, (ii) patients from whom carbapenem-susceptible Klebsiella spp.

Asthmatic Symptoms, Physical Activity, and Overweight in Young Children: A Cohort Study

PEDIATRICS Vol. 121 No. 3 March 2008, pp. e666-e672

Prevalence of asthma and overweight has increased simultaneously during the past decades. Several studies have reported an association between these two health problems, but it is unclear whether this relation is causal. We hypothesize that children with asthmatic symptoms are less physically active, which may contribute to the development of overweight.

Prolonged acute mechanical ventilation, hospital resource utilization, and mortality in the United States

Critical Care Medicine. 36(3):724-730, March 2008

Adjusted costs of mechanical ventilation (MV) are $1,500 per patient-day. We compared the prevalence, characteristics, and outcomes of MV <96 hrs (MV<96) and prolonged acute MV (PAMV) of >=96 hrs' duration in a representative sample of U.S. hospital discharges.

Safety and efficacy of using a surgivac pump for the drainage of chronic indwelling pleural catheters in malignant pleural effusions

Respirology doi:10.1111/j.1440-1843.2008.01238.x

This descriptive study assessed whether a reusable commercially available surgivac pump was a safe and affordable method for draining chronic malignant pleural effusions with an indwelling pleural catheter.

Inhaled Corticosteroids as Additional Treatment in Alpha-1-Antitrypsin-Deficiency-Related COPD

Respiration (DOI: 10.1159/000119718)

No consistent data are available regarding the effect of inhaled corticosteroids (ICS) in alpha1-antitrypsin-deficiency (AATD)-related COPD. Recent data report inflammatory effects of the polymers of alpha1-antitrypsin on the peripheral lung.

Severe Legionella pneumonia successfully treated by independent lung ventilation with intrapulmonary percussive ventilation

Respirology doi:10.1111/j.1440-1843.2007.01220.x

A case of severe Legionella pneumonia was successfully treated by independent lung ventilation (ILV) with intrapulmonary percussive ventilation (IPV).

A Novel Strategy for Treatment of Metastatic Pulmonary Tumors: Radiofrequency Ablation in Conjunction with Surgery

Journal of Thoracic Oncology. 3(3):283-288, March 2008

Local treatment that includes surgical resection of metastatic pulmonary tumors is controversial because of the biologic features and invasiveness of these tumors. We report our experience with a premeditated treatment involving combined computed tomography-guided radiofrequency ablation and surgical resection in three patients with metastatic pulmonary tumors.

Cough Efficacy Is Related to the Disability Status in Patients with Multiple Sclerosis

Respiration (DOI: 10.1159/000119641)

Cough is an important defense mechanism, whose main function is to remove mucus and/or foreign bodies from the airways.

Neural respiratory drive during apnoeic events in obstructive sleep apnoea

Eur Respir J 2008; 31:650-657

For a given neural drive, oesophageal pressure during apnoeic episodes may differ from that during airflow, since inspiratory airflow and increased lung volume both reduce pressure generation. It was, therefore, hypothesised that diaphragm electromyography (EMG) may provide additional data to oesophageal pressure when used for the assessment of neural drive in patients with obstructive sleep apnoea, whose breathing is associated with variable airflow and changes in lung volume.

Interstitial lung diseases in infants and children

Eur Respir J 2008; 31:658-666

Interstitial lung disease (ILD) in infants and children represents a heterogeneous group of respiratory disorders that are mostly chronic and associated with high morbidity and mortality.

Sequestration and homing of bone marrow-derived lineage negative progenitor cells in the lung during pneumococcal pneumonia

Respiratory Research 2008, 9:25

Bone marrow (BM)-derived progenitor cells have been shown to have the potential to differentiate into a diversity of cell types involved in tissue repair.
