- Weekly contribution is based on plan and
status of employment
- Eligibility is the first of the month
following the 3-month provisional period
Life Insurance (Full time
employees only)
- No employee contribution - company pays premium
- Equal to 1-times the annual salary not to exceed $50,000
401K Retirement Plan
- Weekly contribution of 1% to 15% of gross pay
- Company matches $ .20 for each dollar
with $250.00 per year maximum
- Enrollment is the first of each quarter after 3
month provisional period
