Community reentry is at the heart of our mission at Northeast Center for Special Care.  

Community reentry or community reintegration is an interdisciplinary approach.  It includes a strong working relationship between the Interdisciplinary Team, the Resident-Neighbor, their family and the community agencies who will provide the services needed to support the independent living of the person.

Whether it be an acquired brain injury, stroke, spinal cord injury, medical acuity, pulmonary or respiratory condition the focus is always to assist the resident-Neighbor to the point of being able to live at the most integrated setting.

Marty L., one of our Northeast Center for Special Care "Graduates," for nearly four years has lived in the community since his discharge.  He has his own apartment and is employed.  Besides rehabilitation the community reentry staff advocated intensively for his community placement.  Marty (R), stands with Jane Keating, RN, Director of Nursing, and Gerry Brooks, MA, CCC, CBIT, Brain Injury Program Director  during a recent visit to Northeast Center.

We look at every individual who comes to us as having the potential to return to the community - no matter what their medical, neurobehavioral or rehabilitation status is.

This dedication to our mission is what has enabled the talented staff at Northeast Center for Special Care to assist hundreds of our Resident-Neighbor "Graduates" to return to the community.

From its inception in the mid-1990’s and since our opening in March of 1999, the Northeast Center for Special Care mission is the rehabilitation and reentry of our Resident-Neighbors to the most integrated settings. Our mission was set well before the Olmstead vs. L.C. Supreme Court Decision in June of 1999.

Our community reentry staff and social workers work diligently to advocate for our Resident-Neighbors and arrange support services.  We work closely with agencies such as: 

  • New York State Department of Health Home and Community-Based Services Medicaid Waiver for Individuals with Traumatic Brain Injury Program.
  • Office of Mental Retardation and Developmental Disabilities (OMRDD) Home and Community-Based Services Medicaid Waiver program.
  • Local Department of Social Services Offices.
  • Consumer Directed Personal Assistance Programs.
