  • CNA
    We are currently hiring Full Time CNAs for our 7:00 AM - 3:00 PM, 3:00 PM - 11:00 PM and 11:00 PM - 7:00 AM shifts (7.5 hr/5 day/week).

    We offer GREAT benefits, pay rates and weekend shift differentials with no mandating! Advancement opportunities available as well.

    Our evening and overnight shifts come with a $1.50 shift differential and all our weekend shifts have an additional $1.00 differential.

  • Community Re-Entry Specialist
    Requirements:
    • Preferred Social Work Degree (LMSW) or BSW with Skilled Nursing Home experience.
    • Minimum of a Bachelors Degree in related field
    • Knowledge of TBI/NHTD/OPWDD community based programs
    • Knowledge of managed care/insurances
    • Possess strong organizational skills to work with multi-disciplinary professionals
    • Experience with entitlements and community based programs
    • Good writing skills and experience with electronic medical record (EMR)

    As a Community Re-entry Specialist will be an active member of the Social Work Team.
    Responsibilities include intake assessment for all new admissions focusing on their discharge potential. Meet with resident/families or legal guardians to determine goals for discharge. Assist the resident with entitlements, completing applications for specific community programs or services, working with community agencies to enable the resident to safely transition to the community or to the least restrictive environment. Provide ongoing counseling to educate residents/families on challenges in transitioning to the community. Work with all facility departments including rehab, case manager, sister facilities and the business office to assure that resident has all service needed.

    We are currently hiring Full Time LPNs for our 7:00 AM - 7:00 PM or 7:00 PM - 7:00 AM (12 hr/3 day/week) shifts and our 7:00 AM - 3:00 PM, 3:00 PM - 11:00 PM and 11:00 PM - 7:00 AM shifts (8 hr/5 day/week).
